Social Work

Our Social Workers focus on taking advantage of the appropriate and available community resources so that patients have the best chance of staying safely in their own homes. Social workers may identify needed medical equipment or caregiving roles for our patients. Sometimes, Social Workers help coordinate alternative living situations such as nursing homes or assisted living should home not be safe.

“You are truly a God-Send. I so appreciate everything you have done and continue to do for us. I really do not know what I would do without your help and guidance.”
